You can then schedule a new appointment date\/time.<\/p>\n<p><a href=\"#top\">Back to Top<\/a><\/p>\n<h3 id=\"consent\"><strong>Consent Forms<\/strong><\/h3>\n<p>After Scheduling your Flu Vaccine Appointment, please take a moment to <strong>print out the<\/strong> <a href=\"https:\/\/www.rochester.edu\/uhs\/wp-content\/uploads\/2026\/10\/Flu-Consent-26-27-Clinic.pdf\" target=\"_blank\" rel=\"noopener\">consent form<\/a> to complete and bring with you to the Clinic. There will be blank forms available the clinic, however <strong>bringing the completed consent form with you cuts down your wait time<\/strong> considerably. (All students have access to printing resources available through the library.)<\/p>\n<p><a href=\"#top\">Back to Top<\/a><\/p>\n<h3 id=\"VIS\"><strong>Vaccine Information Sheet<\/strong><\/h3>\n<p>For additional information regarding this vaccine, please review the <a href=\"https:\/\/www.rochester.edu\/uhs\/wp-content\/uploads\/2025\/09\/Flu-VIS-01-31-2025.pdf\" target=\"_blank\" rel=\"noopener\">vaccine information sheet<\/a>.<\/p>\n<p>The flu vaccine is <strong>covered in full for all active members of the UR Student Health Insurance Plan (Aetna) and generally covered in full by most private insurance plans<\/strong>. Contact your insurance plan directly for specific coverage details.
